Now a new systematic review, published in the journal Nutrients on June 18, 2026, offers a clearer picture of what this ancient plant compound does inside the gut — and why that may matter for overall health.

Researchers analyzed seven randomized controlled trials involving adults with a range of conditions including type 2 diabetes, high cholesterol, and colorectal adenoma. Study sizes ranged from 34 to 446 participants per intervention arm.

What the Data Shows

Six of the seven studies found meaningful changes in gut microbiome composition following berberine supplementation. In participants with type 2 diabetes, the most notable shifts included increased γ-Proteobacteria and decreased butyrate-producing bacteria. These bacterial changes correlated with improvements in fasting blood sugar, cholesterol levels, and inflammatory markers.

The association held across diverse health conditions, suggesting berberine’s influence on gut bacteria is not limited to a single patient type.

An Important Caveat

The researchers are explicit about the study’s limits. The fact that “microbiome changes and health improvements happened at the same time doesn’t mean one caused the other,” the study notes. Larger trials with more diverse populations are needed before drawing definitive clinical conclusions.
